A number of proposals developed by Integrated Care Partnerships (ICPs) have now been agreed and over £5 million is now being invested by Local Commissioning Groups to allow proposals to be implemented over the next year.
Throughout Northern Ireland, ICPs are now working in local areas to reshape how health and social care services are planned and delivered for frail older people and those with long term conditions.
Over the past year, ICPs have brought together a range of providers from across the health and social care system to review how care is being delivered for each of these patient groups and to consider how services could be improved and better coordinated.

|Western ICP Area – Derry-Londonderry, Limavady and Strabane (Northern Sector); Omagh and Fermanagh (Southern Sector).|
