A patient education programme for people with type 2 diabetes, developed in conjunction with the Northern Integrated Care Partnerships, has won an award.
The DESMOND team from the Northern Health and Social Care Trust won the Innovation award in this year’s Celebrating DESMOND Annual Awards Programme.
According to the judging panel, the team’s entry showed ‘star quality’ and were impressed by their commitment to the DESMOND philosophy and principles.
The DESMOND (Diabetes Education and Self Management for Ongoing and Newly Diagnosed) initiative is a structured programme aimed at providing education and support to patients to help them manage their condition and prevent avoidable complications.
Dr Brian Connor, GP lead for Causeway ICP , said the award was a reflection of the dedication and commitment by the team to establish education programmes for the growing numbers of people in the area with type 2 diabetes and to help them manage their condition.
“This initiative highlights how, alongside our local commissioning colleagues, we can make a real difference to people with diabetes.”
Caroline Finlay, Northern Trust’s DESMOND facilitator, said the award highlighted the importance the courses were making to the lives of those living with type 2 diabetes.
